Note to Athletes:
If you plan to play Division I or II athletics, an NCAA Clearinghouse form must be completed. This form is ONLY available online at: NCAACLEARINGHOUSE.NET. Please complete the form, which requires a payment of $30 with a valid VISA or MasterCard credit card. Once the form and payment information are completed, please continue to the next screen where you will need to print up to 2 copies of the Transcript Release form also on this Web site. Please return the 2 NCAA Clearinghouse Transcript Release forms to Mrs. Berger. Upon receipt of the NCAA Clearinghouse Transcript Release form, the student's transcript will be sent to NCAA.
